Some brain health supplements have legitimate human trial data behind them. Others are expensive placebos. Knowing the difference saves money and may actually support your cognitive health.

The Global Council on Brain Health reviewed the available evidence and reached a clear conclusion: for most healthy people, a nutrient-rich diet beats supplementation. They could not recommend any single ingredient or product for brain health across the general population.

A 2021 randomized, double-blind, placebo-controlled trial of 100 healthy older adults (ages 50 to 85) with age-associated memory impairment found that 500 mg/day of Cognizin® citicoline for 12 weeks significantly improved episodic memory and composite memory scores compared to placebo (p = 0.0025 and p = 0.0052, respectively). A 2023 systematic review and meta-analysis of 7 studies confirmed that citicoline improved cognitive status across patients with mild cognitive impairment, Alzheimer's disease, and post-stroke dementia, with pooled standardized mean differences ranging from 0.56 to 1.57.

This does not mean multivitamins are a substitute for a nutrient-rich diet. But for adults over 50 or those with dietary gaps, a quality multivitamin provides a cost-effective baseline of cognitive support.
Vitamins B6, B12, and B9 (folate) support methylation reactions, homocysteine metabolism, and nerve function. Elevated homocysteine is associated with accelerated brain atrophy and cognitive decline. An Oxford University study (VITACOG trial) found that older adults with mild cognitive impairment who took B vitamins alongside adequate omega-3 status had 73% less brain shrinkage over one year compared to placebo.
The catch: this benefit was concentrated in individuals with elevated homocysteine levels. For people with normal B vitamin status, supplementation did not produce meaningful effects. Test your levels before assuming you need B vitamin supplements.
Observational data consistently shows that people who eat more fatty fish have sharper cognitive function as they age. Omega-3s (DHA and EPA) support neuronal membrane fluidity, reduce neuroinflammation, and promote cerebral blood flow.
